Provision of information pursuant to Act No. 106/1999 Coll., on Free Access to Information.
Act No. 106/1999 Coll., on Free Access to Information, as amended (hereinafter referred to as “the Act”), requires Na Františku Hospital to provide information related to its scope of activities. It also sets forth the conditions under which information is provided and the procedure for handling requests.
Providing Information Upon Request
A request for information under this Act may be submitted orally at Na Františku Hospital or in writing.

Procedure for Submitting and Processing Written Requests
- The application is considered filed on the date it is received by Na Františku Hospital.
- The request must clearly indicate that it is addressed to Na Františku Hospital and that the applicant is seeking information pursuant to this Act.
- An individual must include in the application their first name, last name, date of birth, permanent address, or, if they are not registered for permanent residence, their current address and mailing address, if different from their permanent address or current address.
- A legal entity shall provide its name, identification number, registered office address, and address for service, if different from the registered office address. The address for service may also be an email address.
- If the request is submitted electronically, it must be filed through the electronic submission system of Na Františku Hospital.
If an application does not contain the information listed above, it is not an application within the meaning of this Act.
The deadline for providing the information is
15 days from the date the request is received or from the date it is completed. The deadline for providing the information may be extended for compelling reasons, but by no more than 10 days.
